many of our retirees (not all) received correspondence from Aetna Health
Connection Disease Management.
This
letter states that it is at no cost to you and it is designed to help you
manage your ongoing health conditions you may have. This disease management
program is part of our
This
is a voluntary program that is designed to assist our retirees in our
lives concerning health.
A
personal nurse RN will be assigned to each individual who signs up and that RN
will call you on the phone and discuss health issues with you and how you might
improve your health.
This
contact may be as often as once every 2 weeks or once every 3 months. The RN
will contact you and ask how you are doing and ask if you have any health
concerns.. Example: You may have questions about a certain medication and you
will be sent information on that medication.
As
mentioned earlier this is a voluntary program and if you wish not to
participate that is ok you are not required to.

followed by a short meeting.

out and visit with your fellow retired workers and their spouses. Enjoy a nice
meal and good company.
